The Flashcard gives a snapshot of incidents IRIC has received covering the source of the reports, type of devices reported, suspected problems and health impacts. Additionally we may ask for your help in identifying incident trends (e.g. by interrogating your local risk management system on specific issues) QR codes are included to provide instant access to IRIC’s ”submit a report” and “view safety alerts” webpages on your mobile phone or handheld device
Please share the Flashcard with your colleagues in your health boards and local authorities to encourage more adverse incident reporting both locally and to IRIC. In the meantime please advise if there is anything you would like to see on this monthly Flashcard.
