On Tue 11 April 2023 we advised that there was a problem with IRIC’s Q-Pulse web portal for reporting adverse incidents. The ISSUE IS NOW RESOLVED. Please start using the web portal again for reporting adverse incidents to IRIC. We have taken the opportunity to update the form on the portal so you will notice a few new fields have been added but there is not a huge difference. The form has been updated because IRIC has been working with colleagues across the UK to develop a common dataset for medical device incident reporting. In Scotland the work is supported by Healthcare Improvement Scotland and Datix Scottish User Group. IRIC are one of the first organisations in the UK to adopt the common dataset which will improve the sharing of incident data with MHRA and manufactures
